Caused by releasing software before it’s comprehensively tested.
Unfortunately a sign of the times.
In the 60s and 70s developers had one chance ever 24 hours to compile and run their programs and if they didn’t check properly an entire day was lost.
Nowadays, developers can submit numerous tests per hour and, as a consequence, don’t check or test as thoroughly as they should
In the latter years of my career I worked for a major bank who outsourced much work to India who, unfortunately, didn’t have the same standards as the UK, implemented a change that hadn’t been properly tested and, having failed to take a system backup before implementing, caused no end of problems for customers over a period of 4 days.
Don’t be surprised if we encounter more problems like this!

Standards have fallen throughout every industry, the fast track training is a major cause and the reason us old gits are in demand.
When our generation are gone i foresee major problems, Corporations will not be able to continuously charge top dollar for shoddy goods which is what we have today.
